Who can refer to the hospice?
Referrals are usually made by an individual's GP, Community or Hospital Macmillan Nurse or Senior District Nurse. They will ensure all palliative care options have been considered and discussed with the individual before deciding whether the hospice is the best place for them to receive their care.
If you think you, your loved one, or someone in your care could benefit from our support, please speak to your GP, Macmillan or District Nurse.

How are referrals made?
Referrals are made using our referral form. Please ensure all required information is completed to enable the team to assess the needs efficiently and prioritise the referral.

Completed referral forms can be posted or faxed back to the hospice, (please follow the instructions on the form).
