Transition Information Network - www.transitioninfonetwork.org.uk - This is a brilliant website for people who support disabled young people in transition to adulthood. The aim of this website is to provide information about the transition process that will be useful to professionals and parents/carers. There is also a section for young people that talks about the transition process and gives links to helpful resources.

boy on laptopMoving on up! - www.movingonup.info - This is a great website and is aimed particularly at young people from minority ethnic communities with a learning disability and their supporters and families to help them find out about transition. Although it is especially for people from minority communities with a learning disability, any young person at transition stage may find it helpful.

Directgov - http://www.direct.gov.uk/en/index.htm - Amazingly useful government website detailing all useful public services all in one place. The Young people and Disabled people sections are particularly handy on subjects such as work, education and financial support.

Every Disabled Child Matters - http://www.ncb.org.uk - Every Disabled Child Matters (EDCM) is a campaign to get rights and justice for every disabled child. They work closely with the disabled children's sector and their 34 000 supporters to lobby Government to ensure disabled children and their families have the services and support they need to lead ordinary lives. There is a young campaigners section so you can get involved too!

Channel Mogo - http://www.channelmogo.org/ - A cool website for all young people in Hertfordshire. here you will find lots of great info on things like bullying, disability, jobs, health and relationships. You can also write reviews, take surveys and enter competitions. Also, download great video clips and find out whats going on on the music scene...

Bedfordshire Libraries - http://www.galaxy.bedfordshire.gov.uk/webingres/bedfordshire/vlib/0.teen/home.htm - This is a great website called 'Teen Turf' where you'll find a virtual homework centre, great books to read, life after GCSE's, and book groups (including a Manga comics reading group!)

Mencap - http://www.mencap.org.uk/ - Mencap support thousands of people with a learning disability, and their families and carers, to lead a full and valued life. There are lots of resources on this site including info for children and young people.

Muscular Dystrophy Campaign - http://www.muscular-dystrophy.org/ - The Muscular Dystrophy Campaign leads the fight against muscle disease by funding world-class research; offering free information and advice about care and support; providing grants towards equipment and campaigning to raise awareness and bring about change. There is a fab microsite for teenagers here and a good forum too.
