What is transition?
What is transition?
The term 'transitions' means change. It's the time it takes you to grow from a child into an adult.
Moving from childhood to adulthood services.
It is a time of new opportunities and increased independence.
It usually happens between the ages of 14 and 18, but it takes different people different amounts of time.
Keech can support you and your family to understand the changes that will take place as you become an adult.
We can offer annual reviews that will enable us to design a transition support plan reflecting your needs.
We will do this by following the Keech Transitions Care Pathway.
