How are referrals made?

Who can refer to the hospice?


Referrals are usually made by an individual's Community or Hospital Macmillan Nurse or Senior District Nurse. They will ensure all palliative care options have been considered and discussed with the individual before deciding whether the hospice is the best place for them to receive their care.

If you think you, your loved one, or someone in your care could benefit from our support, please speak to your Macmillan or District Nurse.

How are referrals made?

Referrals are made using the referral forms for our In-patient and Day Hospice services respectively.

These forms can be obtained by telephoning the South Bedfordshire Palliative Care Advice Line on the number above.

Completed referral forms can be posted or faxed back to the hospice, (please follow the instructions on the form).
