Referrals can be made by completing our referral form and sending this to the Children's Community Service Manager at Keech. Please contact them if you would like to discuss the case first.

What happens next?
The families will be asked for permission to obtain further written information from the GP, consultant and other relevant professionals. The referral panel, consisting of at least three people including the Children's Community Service Manager, Hospice Social Worker and a Care Team Member, will base their decision on the information received.
The referral panel is held on the 2nd Monday of each month.
Urgent referrals
In exceptional circumstances i.e. when a child is in the terminal stage, a referral decision can be made by two team members before all the written information is gathered. This allows an immediate service to be offered to the family.
